Recoupment Project: Payments Without Authorization

We have identified claims that were overpaid due to either missing authorization for the date of service or delayed authorization updates. These claims will be subject to recoupment. Providers may submit a post-service appeal within 120 days from the remittance date of the recoupment.

Providers may submit a post-service appeal in one of three ways:

  1. Provider Portal (Preferred Method)
  2. Mail
    • Complete the Appeal Cover Letter (PDF) and include all supporting documentation (e.g., medical records). Send to:
