Disease Management

Meridian has special Disease Management programs to help our members with asthma, diabetes, CVD (heart problems) and COPD (lung problems) stay healthy. 

Meridian's Disease Management program give you access to support from nurses and healthcare staff as well as educational materials and newsletters to help you learn about your disease.

You can join our Disease Management program in one of three ways: 

  1. If you have asthma, diabetes, CVD or COPD, you may be signed up for the program automatically by Meridian.
  2. You can sign up for the Meridian Disease Management program yourself by calling 888-437-0606. You can stop participating at any time.
  3. Your doctor can sign you up for the program.

Meridian's respiratory disease management program covers asthma and chronic obstructive pulmonary disease (COPD) conditions. The goal of the program is to help you manage your asthma or COPD symptoms and keep your disease under control.

Meridian's Diabetes Disease Management program is designed to support the treatment plan your doctor has developed for you and to assist you with controlling your disease.

Meridian's Heart Disease Management program can help you manage heart conditions and prevent long term complications. Our Heart Disease Management program can assist members with heart failure or coronary vascular disease (CVD).

To help our members reach their weight loss goals, Meridian offers services such as nutritional counseling and the Weight Watchers Program to qualifying members. You can call Member Services at 888-437-0606 to learn more about these benefits.
