Michigan Diabetes Prevention Program (MiDPP)
The Michigan Diabetes Prevention Program (MiDPP) is an evidence-based lifestyle change program designed to help members with prediabetes or those at high risk prevent type 2 diabetes.
Eligibility criteria include:
- Enrollment in Michigan Medicaid or Healthy Michigan Plan
- Age 18 or older
- Overweight and not pregnant
- No prior diabetes diagnosis
- Prediabetes-range lab result, history of gestational diabetes, or high-risk score
Participants receive support from a trained lifestyle coach to achieve modest weight loss (5–7%) and increase physical activity (150+ minutes/week). The year-long curriculum includes nutrition, physical activity, stress management, and behavior change strategies.
The MiDPP is offered at no cost to eligible Medicaid members following the Michigan Department of Human Services guidance.
Programs are delivered in-person, online, or via distance learning. Providers are encouraged to refer eligible patients and direct them to local or virtual DPP options.
