Billing Update for Vision Procedure Code V2531

Based on provider feedback, we will be updating the pricing methodology for procedure code V2531. Effective 01/01/2026, V2531 (contact lens, scleral, gas permeable, per lens) will be reimbursed using invoice pricing rather than a percentage of billed charges.

What this means for claims submission:

• All claims submitted for V2531 must include the invoice at the time of submission.
• Claims submitted without an invoice will be denied with a request for the required documentation.

Previously submitted claims:

• If you have already submitted claims for V2531 without an invoice, please submit a replacement claim with the invoice attached.
• Once received, the claim will be reprocessed accordingly.
