Becoming a Meridian Pharmacist

Before Submitting Contract Request:

To begin participation with Meridian and begin seeing Meridian members, providers will need an NPI number and their own Michigan Medicaid Provider ID if they do not have one already. Providers should also verify that their credentials are up to date and accurate in the CAQH Provider Data Portal. This is key in ensuring the credentialing process with Meridian is seamless.

Submit Contract Request to Meridian:

• Visit Meridian’s “Join Our Network” page to review our family of plans and steps to Join Our Network
• Complete the Contract Request Form
• Complete all required fields. Groups and clinics will require a roster submission. Note: Completion of this form does not guarantee inclusion into the provider network. It generally takes 30 business days to analyze the form and make a determination if the contract process will commence. Failure to accurately complete the form will significantly extend this processing time.

Although not required, it is strongly recommended that pharmacist billing Meridian register for the Secure Provider Portal. The provider portal is the most efficient way to submit claims, check claims status, member eligibility, and submit prior authorizations.
