Evolent Prior Authorization Updates Effective January 1, 2027
In partnership with Meridian Medicaid health plan, Ambetter from Meridian Marketplace health plan, and Wellcare Medicare health plan including Wellcare Meridian Dual Align, Evolent is adding the following procedure codes to prior authorization, effective Jan. 1, 2027, as part of our ongoing efforts to improve the prior authorization process for providers and members. These updates will create a more consistent set of prior authorization requirements across health plans.
Each of the affected codes in the following table is managed by Evolent Specialty Services, the utilization management partner of Centene. If you have questions about specific prior authorization codes or how these changes affect your practice, please reach out to your local Provider Engagement representative.
The following Cardiology codes will require prior authorization through Evolent Specialty Services beginning Jan. 1, 2027.
