Update to Professional Liability Documentation Requirements
Dear Providers,
Meridian is committed to maintaining strong audit readiness and aligning with credentialing best practices. As part of these efforts, Meridian is implementing an important update to the professional liability documentation requirements for credentialing and recredentialing.
What’s Changing
Effective May 15, 2026, professional liability attestations will no longer be accepted. All providers must instead submit a current Certificate of Insurance (COI) as part of the credentialing or recredentialing process.
New Documentation Requirements
To meet credentialing standards, the submitted Certificate of Professional Liability Insurance must meet all of the following criteria:
- A valid and current Certificate of Professional Liability Insurance is required for all credentialing and recredentialing submissions.
- The individual practitioner’s name must appear directly on the certificate or on an accompanying endorsement or addendum page.
- The certificate does not need to be linked to a specific practice location; however, it must clearly demonstrate active professional liability coverage for the individual practitioner.
Why This Change Matters
This update is being implemented in response to external audit feedback and is intended to ensure consistent, verifiable documentation of active professional liability coverage across all Meridian markets.
