EX VO – Void Service for Administrative Reasons

Claims may be voided with the EX VO (Void Service for Administrative Reasons) reason code due to various administrative issues. One of the most common causes is incorrect billing of National Provider Identifiers (NPIs) on the claim form.

Common NPI Billing Issues

To help avoid claim voids, please ensure NPIs are billed correctly in the following fields:

  • Box 24J (Rendering Provider NPI):

The NPI reported in Box 24J must be a Type 1 (Individual) NPI.

  • Box 33a (Billing Provider NPI):

The NPI reported in Box 33a must be either:

  • A Type 1 NPI for a Sole Proprietor, or
  • A Type 2 NPI for an Organization.

What Happens If NPIs Are Incorrect?

If NPIs are not billed appropriately in these fields, the claim may be rejected or voided upon receipt using the EX VO reason code.
