Exciting News for Our Valued Partners - Deemer Notifications & Our Brand - New HIDE D-SNP Launching 1, 2026

Dear Providers,

Meridian is excited to announce the launch of a new Highly Integrated Dual Eligible Special Needs Plan (HIDE D-SNP) effective January 1, 2026, as part of Michigan’s MI Coordinated Health Plan. This new product will replace the current MI Health Link Program, which will conclude on December 31, 2025.

As part of our preparation for this new product line, amendments for all product participation were mailed on May 23, 2025, to professional providers with existing direct contracted relationships with Meridian.

What Are My Responsibilities If I Received This Amendment for All Product Participation?

You do not have to take any action if you agree with our amendment for all product participation, which will consider you as PAR (participating) for all our Meridian lines of business under our current direct agreement. Your participation will become effective automatically on July 18, 2025, and will take roughly 30-60 days to configure your new product enrollment in our system. Please allow time for setup before servicing/ billing these new to you, product lines.

If you do not wish to participate with Meridian for all product offerings, please opt-out via email by contacting us at: MIProviderContracting@mimeridian.com. If an opt-out notification is not received by June 22, 2025 to the inbox listed above, you will be deemed into all Meridian Products as par effective July 18, 2025.

Note for PHO/ACO Providers

If you are part of a PHO or ACO type of relationship with us and you received this notice, please contact us as soon as possible through the dedicated inbox so that we could understand if you received this because you may have an active direct relationship conflict with us that we need to resolve. If you have an active direct relationship with Meridian you cannot enter into a PHO or ACO roster relationship with Meridian and this needs to be addressed and resolved together.
