Michigan Meridian Medicaid – Increasing Access to Continuous Glucose Monitor (CGM) Supplies and Prescriber Reminder

Applies To: Medical Benefit (DME/Medical Supply Company) and Pharmacy Claims
Effective July 1, 2026, Michigan Meridian Medicaid will allow certain claims for Continuous Glucose Monitor (CGM) supplies, such as Dexcom or Freestyle Libre CGM system components, to adjudicate when specific clinical and billing criteria are met, for both pharmacy benefit and medical benefit claims. This change is intended to improve timely access to CGM therapy for eligible members with qualifying diagnoses. IMPORTANT NOTE FOR PRESCRIBERS: Please submit your orders through only ONE benefit channel (pharmacy or medical supply/DME) to avoid duplication of services for our members and claim rejections for the servicing providers.
Manual prior authorization is NOT required for the following groups when the patient’s medical or pharmacy claim is submitted with an appropriate diagnosis code (see list of appropriate diagnosis codes at the end of this bulletin).
- Type 1 Diabetes (all ages)
- Type 2 Diabetes (ages 0 through age 5)
- Gestational Diabetes & Diabetes in pregnancy (all ages)
Please note that traditional prior authorization is required for patients outside of these groups according to the standards of coverage and documentation requirements in the Michigan Department of Health and Human Services (MDHHS) Medicaid Provider Manual, Section 2.11.B. Continuous Glucose Monitoring Equipment and Supplies.

- Dexcom G6 (note: manufacturer of Dexcom plans discontinuation 7/1/2026, production will cease for the G6 model/components, but may be available through end of 2026 with existing stock at pharmacies and medical suppliers)

CGM Procedure Codes Included for bypass PA with diagnosis code (Medical Benefit side):
- A4239 : Supply allowance for non-adjunctive, non-implanted continuous glucose monitor (cgm), includes all supplies and accessories, 1 month supply = 1 unit of service
- E2103: Non-adjunctive, non-implanted continuous glucose monitor or receiver
- A4238 : Supply allowance for adjunctive, non-implanted continuous glucose monitor (cgm), includes all supplies and accessories, 1 month supply = 1 unit of service
- E2102: Adjunctive, non-implanted continuous glucose monitor or receiver
- A9276 : Sensor; invasive (e.g., subcutaneous), disposable, for use with non-durable medical equipment interstitial continuous glucose monitoring system, one unit = 1 day supply
- A9277 : Transmitter; external, for use with non-durable medical equipment interstitial continuous glucose monitoring system
Billing Requirements
To qualify for the prior authorization bypass, claims must include a corresponding ICD-10 diagnosis code for the patient and be submitted through only one benefit channel (pharmacy benefit or medical benefit).
Claims submitted without an appropriate diagnosis code or outside the applicable age criteria may be rejected will still require a manual prior authorization. This policy change applies only to the removal of the prior authorization requirement; all other applicable coverage rules, benefit limitations such as quantity limits, and claim edits remain in effect.
Prescriber/Requesting Provider/Servicing Provider Recommendations
- Ensure accurate ICD-10 diagnosis coding on all prescriptions and medical supply orders for continuous glucose monitoring devices/systems/supplies/components
- Confirm member age eligibility at the time of billing
- Submit claims through the appropriate benefit channel (pharmacy or medical)
