Preferred Diabetic Test Strips and Glucometer Changes (Effective 10/1/2025)

Meridian would like to inform all prescribers and pharmacies that the coverage of OneTouch Diabetic Test Strips is changing on 10/01/2025 for all members. 

Effective 10/01/2025, OneTouch Diabetic Test Strips will be removed from the formulary (list of covered medications). Starting on 10/01/2025, the preferred diabetic test strips and glucometers will be Accu-Chek Guide Test Strips, along with Accu-Chek Guide and Guide Me branded glucometers. This applies under the pharmacy benefit for Michigan Meridian Medicaid members. Please refer to the table below for information regarding medication changes and alternative agents.

Meridian would like to work with you to help transition impacted members to a formulary alternative as soon as possible. The upcoming preferred alternative Accu-Chek Guide Test Strips and Glucometers will be available for coverage starting 07/07/2025. For Meridian Medicaid members, you may start prescribing and dispensing the Accu-Chek Guide test strips and glucometers during this transitional period from 07/07/2025 to 10/01/2025.

For Meridian Medicaid members, please ensure that prescriptions for diabetic test strips and glucometers are written and dispensed exclusively for the Accu-Chek Guide brand from 07/07/2025 going forward. Dispensing pharmacies and members are also being notified directly of this upcoming change.

If a member requires continued therapy for a product that has been changed, please submit a prior authorization with appropriate clinical documentation. Prior authorizations can be submitted electronically on www.covermymeds.com by selecting from the menu options, or by fax to 877-355-8070. For fax requests, a blank Medication Prior Authorization Request Form can be found at https://www.mimeridian.com/providers/pharmacy.html .
