As of November 3rd, 2025, providers are now required to attest to the following below for CSHCS eligibility:
- Is willing to accept new Enrollees in CSHCS with potentially complex health conditions
- Regularly serves children or youth with complex chronic health conditions
- Has a mechanism to identify children/youth with chronic health conditions
- Provides expanded appointments when children have complex needs and require more time
- Has experience coordinating care for children who see multiple professionals (pediatric subspecialists, physical therapists, mental health professionals, etc.)
- Has a designated professional responsible for care coordination for children who see multiple professionals
- Provides services appropriate for Health Care Transition, including but not limited to; the use of transition readiness assessment and adoption of transition policy that is publicly posted and specifies:
- The transition time frame
- Transition approach
- Legal changes that take place in privacy and consent at age 18
Note: Provider must maintain a roster of practitioners who meet the criteria listed above and able to serve Enrollees in CSHCS. Providers must also annually survey network PCPs serving adults to assess availability of these practitioners to serve special populations of transition-aged youth and young adults, including Enrollees in CSHCS with chronic health conditions.
