Meridian Medicaid Drug List Changes starting 10/1/2026

Dear Meridian Member,

Your health care and access to medications are a priority. On 10/1/2026, there will be changes to the Michigan Meridian Medicaid drug list. The preferred drug list (also known as the formulary) is a list of medications covered by your plan.

THE FOLLOWING DRUGS WILL HAVE A CHANGE IN STATUS STARTING ON 10/1/2026.

Non-preferred Medication with Prior authorization required

Quantity Limit of 1 tablet per day

Zymfentra (infliximab-dyyb) pen auto-injector

Quantity limit updated per package size change:

New Quantity limit of 2 pens per 28 days, replaces the previous limit of 1 pen per 28 days

Remains non-preferred with prior authorization required

Zymfentra (infliximab-dyyb) prefilled syringe

Added new Quantity Limit of 2 syringes per 28 days

Remains non-preferred with prior authorization required

Linzess (linaclotide) capsules

[72mcg, 145mcg, 290mcg]

Removed clinical prior authorization requirement

Preferred medication, no prior authorization  required

Age Limit (minimum age of 2) remains applicable

Quantity Limit (1 capsule per day) remains applicable

Lubiprostone capsules

[8mcg, 24mcg]

Removed clinical prior authorization requirement

Preferred medication, no prior authorization  required

Age Limit (minimum age of 18) remains applicable

Quantity Limit (2 capsules per day) remains applicable

You can talk to your doctor to ask if this change impacts you. If there are new restrictions or your drug has been removed, there may be other medications you can take instead. There are two ways that you or your doctor can find other medications covered by Meridian:

  • Look on our website at mimeridian.com. Under “Select Your Plan” choose Meridian Medicaid plan. Under “Benefits and Services” choose Pharmacy. Click on the link “Formulary Search” to search for a medication. The search tool shows covered medications with any restrictions that may apply.
  • Call Member Services at 888-437-0606 (TTY: 711). Our phone lines are open 24 hours a day, seven days a week.
